How the attendant application is linked
The attendant category is connected to the patient's medical travel. Application details should match the patient's passport, e-Medical application and hospital letter. The official portal sets the current fields and document format.
Regular Medical Attendant or MED-X routes may differ from the electronic category. Applicants who are not eligible for e-Visa should follow the relevant Indian mission's process.
How many attendants are permitted?
The official e-Visa FAQ allows up to two e-Medical Attendant visas against one e-Medical visa. Each application still has to meet the live requirements.[1] Permission for two companions does not mean two are needed.
The useful question is what help the patient will need. Luggage, meals and ordinary transport are different from skilled nursing or a medical escort. The clinical team should explain the expected help before the family books travel.
The duration wording needs confirmation
The official page's main table currently lists one year from arrival with multiple entry for the e-Medical Attendant category. An older FAQ on that same page still says 60 days. Check the current application and the terms printed on the granted ETA.[1]
Why a companion can be useful
After joint replacement or ACL surgery, a family member can manage luggage, keep documents together and help with airport transfers. During the hospital discussion, they can listen, take notes and learn the medication and discharge plan.
In local accommodation, they may help with meals, transport and practical tasks. Their presence is particularly useful during a multi-leg return journey.
Prepare the companion for discharge
The companion should hear the medicine, wound, walking-aid and emergency instructions with the patient. They can keep the discharge pack together and note the contact route for routine questions. They should also know which warning symptoms require local emergency care.
Divide responsibilities clearly
- Before admission: help organise records, transport and accommodation.
- In hospital: listen to instructions and clarify the practical home plan.
- After discharge: assist with meals, documents, ordinary transport and the safe use of the accommodation.
- During travel: handle luggage and check-in while the patient uses the confirmed assistance.
- After return: help the patient reach planned local care and recognise warning symptoms.
What an attendant is not
A relative is not a replacement for nursing, physiotherapy or medical assessment. They should not lift unsafely, change wound care without instruction or make medication decisions. A patient who requires skilled nursing or a medical escort needs a separate plan.
Documents to prepare
Each companion needs their own entry permission. For foreign nationals, including OCI holders, the e-Arrival card is an additional submission within 72 hours before arrival; a patient's form does not cover them.[4]
- Attendant passport and required photograph
- Patient passport and e-Medical application details
- Hospital letter and treatment information
- Relationship or other evidence requested by the live process
- Travel, accommodation and contact details
- Copies of approvals and, for each applicable traveller, the saved e-Arrival confirmation
Budget separately
The standard surgical package covers the patient's planned hospital admission. It does not include the companion's visa, flight, meals, accommodation or missed work. These can be included only when a custom arrangement lists them.
Add insurance, flexible return arrangements and any second room or extra bed to the trip budget. If the patient's stay is extended, the companion may also need to change flights and leave from work.
Choose the companion for the job
The most helpful person is calm, physically able to assist with ordinary tasks, comfortable managing documents and willing to follow clinical instructions. They should be available through the local recovery period and understand the emergency plan after returning home.
Plan for the companion's own needs
Long hospital days, unfamiliar transport and interrupted sleep can be tiring. Confirm accommodation, meals, medicines, insurance and return flexibility for the attendant as well. A companion who becomes unwell cannot provide reliable practical support.
If two attendants are eligible, that does not mean both must travel. Balance useful support against extra cost and accommodation.
When a family companion is not enough
A patient who needs skilled nursing, oxygen, complex transfers or clinical monitoring requires a professional plan. A relative should not attempt a lift that puts both people at risk. Airline medical escort or nursing arrangements, when needed, are separate from the attendant visa.
Discuss these needs before travel. Discovering at discharge that the patient cannot use the planned room or manage the airport can cause an avoidable delay.
Check the live rule before paying
Confirm the validity and entries shown in the current application and granted ETA. Government rules and nationality eligibility can change, and approval cannot be guaranteed by the hospital or surgeon.
Read the medical visa guide and apply only through the official Government of India route.
Travel with a shared document pack
Both travellers should have the hospital address, routine contact route, passport and visa copies, accommodation details and emergency numbers. The patient should keep personal medicines immediately accessible. The companion can hold a duplicate discharge pack in case a device or bag is lost.
Questions before choosing an attendant
- Does the companion have the correct passport, visa or OCI documents and their own e-Arrival submission where required?
- Can they stay through the expected local recovery period?
- Can they manage luggage and ordinary transfers safely?
- Do they understand that clinical care belongs to professionals?
- Are their flight, accommodation and insurance costs budgeted?
- Do they know the emergency and routine contact routes?
